Filling these out at home saves about 30 minutes at your first visit.
How to send them
Print the forms, fill them in by hand, and either bring them with you or send them ahead by email to [email protected]. A clear photo of each page from your phone is fine.
If you'd rather do the paperwork here, arrive 30 minutes before your appointment. We take X-rays at the first visit too, so allow time for that.
What to bring
- Any dentures or partials you wear now, even if they are broken or you no longer use them.
- A list of your medications.
- Your insurance cards, dental and medical.
- If you were referred, anything your dentist, surgeon, or the VA gave you.
